What is a Brand name Drug and what is a Generic Drug

Why Brand Name Drugs?

While FDA is right in approving the pharmacological effects of the generic drugs to the brand name drugs, clinicians in all medical specialties are well aware the manner patients react to the changes in the drugs. One recent example was the change of brand name Keppra to generic Levitracetam, Depakote to Valproic acid,  or Lamictal to Lamotrogine for epileptic patients. There was sudden surge in urgent office visits and emergency room visits. I hope FDA or the proponent of the brand name drugs calculate the cost associated with such a surge. Breakthrough seizures disappeared when the brand name was restored.

This week I had a patient who is taking Topax for migraine prophylaxis. she notice that her headaches are not better control and she found out that her Topamax does not look the same and forced the Pharmacist to change it to brand name and her headches were better controlled again. Now remember in this case the patient was willing to save money but she could not do so as her better headache control was more important.

There are millions of examples that can be sited by patients and physicians alike but I have a different reason to defend the brand name drugs.

I came to USA with a dream and it also included dream of best healthcare. Since Mr. Bush introduced the Medicare D and soon after AMA and AARP endorsed drug coverage, patients are in constant problem of getting their medicine and Insuarnce companies and other drug coverage plans are making millions a day. If we were honest caregiver we should have given all the money to the patients and not like a, B or Insurance companies to make money out of their pennies. Any way, thats the time when I started paying attention to the whole issue becayse it was not as bad prior to that.

Many times when a physician wrote a drug and insurance company will force the physician to change into generic. This practice is still very much alive and progressively getting worse.

TODAYS GENERICS WERE YESTERDAY'S BRAND NAME DRUGS:

Few months ago NBC hosted a man from AARP who was telling people to buy generic drugs. (The same NBC's physician told people not to go to ER if they have swine flu symptoms). I am not sure what NBC was trying to promote but we should be careful what we tell people. If we as a modern nation become genric lover drugs then we should pack our bags and move to some third world country. I smell from Obama admnistration the same thing what that AARP man told at the NBC. I would let every one know that discouraging brand name drugs will take us 100 year back. We are supposed to be the leader and the best in the world but discouraging innovation will kill the future of new drugs. Thre will bo no generic tomorrow if there is no brand name today. We should try to cut the funding from war and bring resources to encourage research and innovation. 

had we not supported Propranolol many years ago there would be no Inderal today.  If there was no Depakote yesterday then there would no Valproic acid today. we have about 20 new gennrics for seizure disorder today compared to about 4 in mid 1990's. All of these generics were brand name yesterday. We should not be selfish and should not ruin the future of our children.

I urge all the physicians to defnd the innovation and the best defence is to support them. At the same time, I will urge the new drug manufacturers to be generous and keep their price reasonably affordable.

I must mention an example of generic drugs. I had to buy Promethazine 25 mg #40 for my wife. I paid $25.00 as copay. I was sort of shocked by the price. I rarely had to buy medicien for myself so this experience was like putting my feet in patient's shoes. As a curiosity I asked teh Pharmacist that how much is my insuarnace going to pay and she told me $27.00. I know that I have not been to pakistan for long time but with maximum inflation and currency devaluation, I am cvertain that I could buy same qauntity of the same drug and probably of same origin (China) in about 50 Cents. The generic drugs is big money making thing for the pharmacies its not chaep for the patient in many cases.

Two Gene Locations Linked To Multiple Sclerosis Discovered

ScienceDaily (June 16, 2009) — Australian and New Zealand researchers have accelerated research into Multiple Sclerosis by discovering two new locations of genes which will help to unravel the causes of MS and other autoimmune disease.

Their findings will be published today in the journal Nature Genetics.

"For decades the cause of MS has remained a mystery. This discovery reveals important new insights into the genetic susceptibility to the disease, "says Professor Trevor Kilpatrick, Director for Neurosciences at the University of Melbourne, who with Dr Justin Rubio of Florey Neurosciences Institutes coordinated the international study.

"The newly discovered gene locations in chromosomes 12 and 20, offer very promising targets which indicate susceptibility to MS," says Professor Kilpatrick.

"They also reveal a link between genetic susceptibility to MS and other autoimmune diseases including Type 1 diabetes, Rheumatoid Arthritis and Graves' Disease and the also the potential involvement of Vitamin D metabolism in the risk of developing these diseases."

"These results are like the key in the door – leading us to where to look for MS susceptibility," explains Professor Trevor Kilpatrick.

The research was conducted by members of the ANZgene consortium, more than 40 investigators from 11 institutions in Australia and New Zealand.

The three year study utilized the MS Research Australia (MSRA) Gene Bank and involved scanning the DNA of 1,618 people with MS and 3,413 people without MS (controls).

Using a genome-wide association scan (GWAS), researchers scanned the entire human genome in broad brushstrokes; looking at genetic landmarks in the genome and then progressively narrowing down their search to individual genes.

Dr Justin Rubio who coordinated the GWAS says these genetic discoveries are a major advance for the field.

"We expect that within one to two years we will be able to fine map these new regions and identify the genetic changes that underpin these findings," says Dr Rubio.

"Our next steps include studying how changes in these target genes might influence the development of MS. This work could provide insight into the development of novel therapeutics," says Dr Rubio.

MS affects some 2.5 million people worldwide and almost 20,000 in Australia. It is a devastating autoimmune disease as it occurs at the prime of life and mostly in young Caucasian women.

"This Australasian team is competing on a global scale to unravel the complex genetics of MS. This is a significant discovery" says Professor Jim Wiley, Chairman of the ANZGene consortium.

Mr Jeremy Wright, Executive Director of MS Research Australia, says: "We are thrilled to have been funding this study with the Australian Research Council and helping in its coordination. It is central to our mission of accelerating MS research to identify susceptibility in individuals so that we can potentially prevent the onset of the disease, and develop better ways to treat it".
